Student Medical Request Form
When the need arises for students to be administered medication during school hours, the following guidelines will be followed:
- Medication will only be administered by staff when written permission is provided by parents/guardians.
- This can be provided by completing and returning the Medical Authority From found below.
- Parents/guardians are responsible for keeping the school up to date with medical information about their children through PAM.
- All medication must be provided in the original packaging.
- For medications associated with Anaphylaxis and Asthma, authority is provided through the up to date Medical Action Plan from a GP.
